In partnership with the North East LHIN, the Change Foundation is hosting a Health Service Provider
Learning Session on Patient Engagements

Wednesday, May 7 - 8:30 a.m. – 3:00 p.m.
Algoma Public Health Unit: 294 Willow Ave, Sault Ste. Marie, ON
(OTN System # 35043810)

Delegates will be engaged in presentations that highlight how to incorporate the voices and experiences of patients/clients/people in their decision making to deliver quality health care services.

Participants will leave with information and tools that will help them to better engage patients in successful health system transformation initiatives, such as Health Links, Health Hubs, and other models of patient-centered care.

While OTN is available, we encourage participants to attend in person.  Face-to-face will allow for a more engaged experience and two-way conversation.

There is no registration fee to attend this HSP forum, however registration is mandatory.
